Opting out of sharing your confidential patient information
Following the recent articles in the press if you want to opt out of sharing anonymised information you will need to do the following;
1) Complete this form (link //nhs-prod.global.ssl.fastly.net/binaries/content/assets/website-assets/data-and-information/data-collections/general-practice-data-for-planning-and-research/type-1-opt-out-form.docx) and return to the surgery to opt out at a local level
2) Visit https: www.nhs.uk/your-nhs-data-matters/manage-your-choice/ where you can opt out at a national level either by completing an online form or by telephoning the NHS Digital Contact Centre phone number 0300 303 5678 – more details are provided in the link
